Acute Home Based Treatment - Taranaki District Health Board

This initiative, established in 2007 provides a crisis resolution service based in the Service Users home setting, specifically targeted at those acutely unwell clients who would otherwise require admission and treatment in an in-patient facility.
It is an approach which is aimed to be responsive, focused, recovery orientated and culturally appropriate.
The service offers a mobile rapid response with frequent home visits, medication management and Psychiatrist involvement.
It also allows for the potential earlier discharge from the inpatient unit to the client's own home environment.
The benefits of the service to the Service User and their Family / Whanau are:
- That it minimizes disruption to the Service user and her/his Family/Whanau
- Empowers the Service User and their Family/Whanau in determining their treatment and recovery

The involvement of a multi discipline and multi agency approach to care.
Referrals are accepted from clinical staff within the Taranaki District Health Board's Adult Mental Health and Addictions Unit and the Services for Older People Team.
